Requests for Qualifications (RFQ*)

IN-HOME PERSONAL CARE AND RESPITE SERVICES

Qualified Service Providers in Southwest Washington

The Area Agency on Aging and Disabilities of Southwest Washington (AAADSW) is designated by the State of Washington as the Area Agency on Aging to serve Clark, Cowlitz, Klickitat, Skamania and Wahkiakum Counties. AAADSW plans, advocates, develops, coordinates and supports a comprehensive service delivery system in the region for persons who are 60 years of age or older, persons with disabilities who are 18 years of age or older, and caregivers of aging and/or disabled persons. Health Care Agencies contracting with AAADSW are overseen by the agency (see AAADSW’s web site at http://www.HelpingElders.org).

RFQ Announcement: This notice represents a formal Request for Qualifications (RFQ) seeking qualified organizations to provide in-home personal care and respite services consistent with WAC 388-106 (see

http://www.leg.wa.gov/CodeReviser/WACArchive/Documents/2012/WAC-388-106-CHAPTER.pdf). The contracts period associated with this RFQ is July 1, 2014 through June 30, 2015. After the initial year, contracts may be renewed annually for an additional three years depending on funding availability and/or contractor performance. Selected contractors must have a facility in the AAADSW service area(s) in which the home care will be provided (i.e., Clark County, Cowlitz and Wahkiakum Counties, and Klickitat and Skamania Counties).

Eligibility: Eligibility requirements are detailed in the RFQ. At a minimum, prospective applicants must:

1. Be a verifiable legal public or private entity, holding a valid Washington State Business License;

2. Have at least three (3) years of experience providing in-home care services described in the RFQ;

3. Comply with all applicable local, state, and federal laws, regulations, policies and contract requirements.

Additional eligibility requirements will be presented in the RFQ. Fiscal and management references, independent financial audits for the past two years, and other screening information will be required.

Organizations that have been debarred, suspended, proposed for debarment, declared ineligible or excluded from participating in a contract by any federal or Washington State agency will not be considered.

Mandatory requirement to apply (RFQ Packet): Entities interested in responding to this notice must request a mandatory RFQ packet, which details all aspects of the application process. This requirement can be met by sending a request to AAADSWcontracts@dshs.wa.gov, and specify in the subject heading “In-Home Personal Care and Respite Services RFQ,” or by submitting a brief written request for the “In-Home Personal Care and Respite Services RFQ” to the following address:

Please be sure to include a contact person, and an email address, current return mailing address, or other preferred method of receiving the RFQ packet. Once all requests have been received the RFQ packets will be distributed (expect a seven day delay to receive a packet through the U.S. Postal Service). Because of our limited staff resources, faxed requests will not be considered, and we are unable to respond to telephone inquiries (opportunities for submitting questions are described in the RFQ packet).

Deadline: Interested entities must request a mandatory RFQ packet by 2:00 P.M., Friday, March 7, 2014 in order to be considered (requests received after this deadline will not be considered).
