Southwest Washington Agency on Aging and Disabilities (SWAAD) is part of the national network of Area Agencies on Aging, a public service agency dedicated to helping persons aged 60 and over, adults with disabilities and their families to access needed community services in Clark, Cowlitz, Wahkiakum, Klickitat and Skamania counties.
SWAAD is seeking to contract with organizations who are licensed in the State of Washington to provide services to secure help in an emergency through an electronic device that is connected to the Client's phone or is a GSM Cellular Unit and is programmed to signal a response center, staffed by trained professionals who will immediately summon help for the Client. PERS may also include medication reminders. PERS does not include wellness checks, smoke alarms or other enhancements made available by the contractor.
The parties to an awarded contract are State of Washington Department of Social and Health Services (DSHS) and the contracting individual or company. SWAAD is not part of DSHS, but is responsible for verifying organizational qualifications, executing the contract and monitoring for contract compliance.
Medicaid Waiver Personal Emergency Response System contracts are procured typically every 3 to 4 years. Contracts that may be awarded are currently scheduled to begin July 1, 2012 through June 30, 2015. Please note that the scopes of Medicaid Waiver services may changed based upon funding levels that are available to DSHS.
Interested parties may request a scanned copy of a sample contract for review, Qualified Applicant Requirements and a List of Disqualifying Crimes by sending an email with the subject line “Request for Information – Environmental Modifications” to SwaadContractsGroup@dshs.wa.gov. Please include the complete name of your business, the contact person, contact phone number, email address and mailing address.
