Request for

Qualifications

In-Home Personal Care and Respite Services

Southwest Washington Agency on Aging and Disabilities (SWAAD) is designated by the State of Washington as the Area

Agency on Aging to serve the five counties of Southwest Washington: Clark, Cowlitz, Wahkiakum, Klickitat, and

Skamania Counties. The Agency plans, advocates for, develops, coordinates and supports a comprehensive service

delivery system in the region for persons who are 60 years of age or older and for persons with disabilities who are

18 years of age or older. More information about SWAAD can be found at our website http://www.helpingelders.org.

SWAAD administers a variety of state and federal funds for in-home services which include personal care services for

DSHS/SWAAD clients and respite services for caregivers. The purpose of this Request for Qualifications (RFQ)

process is to review organizations’ qualifications to deliver high quality, safe and dependable in-home services to

eligible clients in each of the five counties.

The contract period is currently planned for July 1, 2010 through June 30, 2011 with three (3) annual renewals

possible. Please note that funding levels may change or be eliminated if federal or state funds are no longer

available to SWAAD.

To request an RFQ packet, organizations must state in writing that they meet the following requirements:

1. The organization is not presently debarred, suspended, proposed for debarment, declared ineligible, or

voluntarily excluded from participating in federal contracts by any federal department or agency.

2. The organization has at least three (3) years experience in Washington State as a licensed in-home service

provider in the home care agency category.

3. The organization has independent financial audit(s) without findings covering the most recent two-year

period. The audit(s) must be conducted by a licensed Certified Public Accountant (CPA) or a recognized financial

firm.

Other requirements and requests for information will be evaluated in organizations’ RFQ Proposals.

that the organization meets the three requirements listed in this Notice.

Requests received after 4:00pm PST on Friday, January 29, 2010 will not be accepted.
