REQUEST FOR PROPOSALS FOR

RESIDENTIAL REFRIGERATOR AND FREEZER DECOMMISSIONING AND RECYCLING

SERVICES FOR PUBLIC UTILITY DISTRICT

NO. 1 OF WAHKIAKUM COUNTY, WASHINGTON

 In accordance with Chapter 39.04.155 Revised Code of Washington, Public Utility District No. One of Wahkiakum County, Washington, is inviting statements of qualifications and performance information from firms interested in providing residential refrigerator and freezer decommissioning and recycling services. Responses must be provided to the District by 4:00 p.m. on Friday, January 15, 2010.

 SCOPE OF WORK shall include, but not be limited to, receiving calls from the District's customers and scheduling appliance pickup; picking up the refrigerators and freezers to be recycled from homes within the Wahkiakum PUD's service territory; verifying that existing refrigerators or freezers to be recycled are functional and are a minimum of 10 cubic feet in capacity; decommissioning and recycling refrigerators and freezers; verifying to the District that recycled units have been decommissioned and that the components have been recycled; completing all paperwork required under Bonneville Power Administration regulations and guidelines.  The District maintains a Small Works Roster pursuant to the provisions of RCW 39.04.155. The Small Works Roster may be used to secure price quotations and to award contracts for various public works projects under $300,000.00 in value, exclusive of sales tax.

 Firms who desire to have their names placed on the Small Works Roster of Public Utility District No. One of Wahkiakum County should notify the District and obtain a Small Works Roster Application form which is required to be completed and submitted in writing. Applicants where required by law, must be properly licensed and bonded to perform work in the State of Washington. The District may be contacted at (360) 795-3266 or by mail to P.O.
